To join our practice, please follow the below steps:

STEP 1: Pre Register your child by clicking HERE. (Click on New to Practice > Register Here located on the bottom left of the page)

You will need to complete a Pre Registration for each child.

STEP 2: Please call our office at (703) 670-0300 to have the New Patient documents emailed to you. You will need to set up a time to come IN PERSON to complete the Registration Form and have your child's copy of last Well Visit, Immunization Record, Insurance card and Parent ID scanned in.

*Appointments will ONLY be scheduled once Steps 1 and 2 have been completed. Pre Registrations that have not completed Step 2 within 30 days will be deleted from the system.

Expectant Parents: Please pre-register using Mom's or Primary Caregiver's Name and Date of Birth. Once baby is born, we will transfer the profile over to Baby's information. Please call the office to have New Patient documents emailed to you and to confirm your due date in order to keep your profile active.

*PLEASE VERIFY WITH YOUR INSURANCE IF OUR OFFICE IS IN NETWORK WITH YOUR PLAN PRIOR TO YOUR FIRST APPOINTMENT. For a list of plsns we are currently accepting, please visit our Billing & Insurance Page.

Our office is Pro Vaccination and follows the American Academy of Pediatrics Immunization Schedule.

To facilitate the check in process, we ask that you arrive at least 20 minutes prior to your scheduled appointment time in order to complete/review forms and to verify your insurance.

Items Required at Your First Appointment: Registration Form (Signed and Dated for the Date of First Visit) Child's Insurance Card Parent's Insurance Card Parent/Caretaker Photo ID Prior Medical Records including Immunization Record (Please verify with your insurance if we are in network with your plan PRIOR to your visit.)

A Legal Parent or Guardian* must be present for the first visit, no exceptions (*Legal Guardians will need to bring Court authorized & signed/stamped Custody Papers.)

Newborns: All of the above Items Required at Your First Appointment, Hospital Discharge Papers Hearing Screen Hepatitis B Vaccine Administration Record (if administered)

*Please call your insurance prior to your first appointment to verify that our office is in network with your plan.

Appointment Policy: Our office requires a minimum of 24 hours notice for cancellation of an appointment. Failure to arrive for your first appointment at our office will result in a discharge from the practice and no future appointments will be made. No exceptions.

Newborn Billing Policy: We will try our best to utilize your insurance for your baby's first visit. In the event that we can not determine active coverage, we will ask that you pay for the visit and will be reimbursed minus a $20 administration fee (including any copays/deductibles) once the baby has been added to your plan and insurance has validated the visit.

Vaccination Policy: Our office firmly believes in the safety and effectiveness of vaccines therefore we are Pro Vaccination. As a medical care provider, we are responsible for maintaining standards assigned to us by the CDC & AAP therefore we have chosen to no longer accept patients whose caregivers refuse essential primary vaccines for non-medical reasons.

Our office offers a Virtual New Parent Tour for expecting parents. Virtual calls are typically done on Wednesdays or Thursday at Noon via Facetime or Google Duo and depends on the availibility of Dr. Gohel.
