Our Billing Policies



Billing & Payment Policy





Our office will submit claims to your insurance company based on the information provided to our facility at the time of registration. Information given to us by your insurance is not a guarantee of their payment and all claims submitted are subject to review by your insurance company. Every patient’s insurance policy is different and it is not possible for our staff to know the limitations of every individual plan. If you have any questions about your coverage, please contact your insurance company's Member Services line directly. Insurance must be an active plan for the date of service with the patient added to the plan as either a dependent or subscriber AND our office must be IN NETWORK with the policy. All copayment/deductible information given is just an estimate, and subject to the terms and limitations of your plan at the time of service.


It is your responsibility to inform our office of any changes in insurance, address, telephone number and email address. It is also your responsibility to ensure your insurance coverage is active, our office is in network with your plan and a Coordination of Benefits has been done on your part on an annual basis. We will do our best to receive full payment from your insurance, but in the event they do not pay or a retraction is made by your insurance, you will be responsible for the balance.


Payment is expected at the time services are rendered. This means we will collect your copay, deductible, or complete payment/balance on file in full at the time your child is seen in our office. Account balances over 60 days will be subject to a collections fee and sent to a collections company for remittance and possible discharge from the practice.


We accept cash, credit card (Debit, MasterCard, Visa and Discover), Apple Pay, Google Pay and online banking checks (payable to Boulevard Center Pediatrics, include your child's account number from your statement).


A Fee will be applied for all returned checks.

Appointment policies





All Office Visits are Scheduled, our office does not accept Walk Ins. Same day appointments are always available, please call to schedule.


Due to scheduling constraints, Well Visits are conducted separately from any Sick or Chronic Care visits.


Requesting or approving treatment for an acute or chronic illness during a Well Visit will be assessed a copay or deductible per the parameters of your insurance company.


If your child is sick during their well visit, the illness will be addressed and the well visit will be rescheduled to another day.


No Food/Drinks are allowed on the premises.

Photography/Video Recording is STRICTLY PROHIBITED on premises due to HIPAA.

Cell Phone Use is Prohibited in Examination Rooms.

Please be mindful of your fellow patrons and the Front Desk: keep the volume of your devices at a low level

*There is a Diaper Genie in the Restroom, do not leave Dirty Diapers in the Examination Room


A Legal Parent or Guardian* must be present for the first visit, no exceptions (*Legal Guardians will need to bring Court authorized & signed/stamped Custody Papers.)



 Our office requires a minimum 24 hours notice for the cancellation or rescheduling of an appointment. There is a $25 No Show/Late Cancellation Fee for each missed appointment. After the 3rd No Show/Late Cancellation, your family will be discharged from the practice.



COVID GUIDELINES FOR IN PERSON VISITS: If you or your child has been exposed to a Positive COVID case within the past 5 day or if you have tested Positive with an At Home COVID Test, PLEASE INFORM THE OFFICE WHEN SCHEDULING YOUR APPOINTMENT, so that we may take appropriate measures to prep the staff, patients and office for your visit. PLEASE DO NOT WAIT TO INFORM US OF YOUR CLOSE CONTACT EXPOSURE OR TESTING POSITIVE AFTER YOU HAVE ENTERED THE OFFICE. For the safety and health of everyone on site, we will conduct appropriate isolation measures for your visit. Only 1 adult is allowed in the office with the sick patient(s.) medicaid billing policy





Please note, a private/commercial insurance will always be considered a child's primary insurance over any type of Medicaid insurance per DMAS.


Failure to notify the office of any other primary insurance or MCO associated with your Medicaid policy will cause your account to become delinquent and will be subject to an administration fee PLUS responsibility for all billed visits.


Please ensure Dr. Gohel is listed as your child's PCP PRIOR to your appointment.


Medicaid visits at our facility are for children ages 18 and under only.


If you intend to use Medicaid as a PRIMARY insurance, please ensure you have no other active insurance coverage.


If you intend to use Medicaid as s SECONDARY insurance, please ensure Coordination of Benefits has been done with both insurances.





Newborn Billing policy





Our office will try our best to utilize your insurance for your baby's first visit.


In the event that we can not determine active coverage, we will ask that you pay for the visit and will be reimbursed minus an administration fee (including any copays/deductibles) once the baby has been added to your plan and insurance has validated the visit.


We kindly ask that you ensure baby has been added to an insurance plan by the time they are 1 month old.


*Please verify with your plan that our office is In Network with your plan PRIOR to your first visit.
